What Can Be Done About Holes In The Subfloor Near The Floors Plugs?

The company installed all the flooring upstairs today and the manager told me that leaving several inches in front of the floor plugs there was a rectangular space cut out of the subfloors, at about the same size as the cork. He said they just put the floor on it and if it gave us trouble they should cut that part of the vinyl out and put a metal plate, similar to what's around the floor plugs, but that wouldn't would only serve as something to plug the hole in the subfloor. I guess I didn't really understand why this would be a problem, but earlier when I walked past the floor plugs in the playroom, the board collapsed and my foot went in!

The floor of my log house in the 80's was made of 2 x 8 T&G pine boxcar flooring and had holes where the metal brackets were placed to hold it in the boxcars. After installing the flooring, I went back, drilled the holes to a nominal size, cut plugs, glued them in, cut them flush, and finished the floor. The finished product looked like it had been staked. The flooring finisher said if the house ever caught fire, smoke and flames could be seen from Spartanburg, South Carolina, due to the heart of the pine.
